Mounjaro vs Ozempic — Which Should You Take?

Mounjaro (tirzepatide) acts on two receptors, GIP and GLP-1, and produced about 15–22% weight loss in trials; Ozempic (semaglutide) acts on GLP-1 alone and produced about 10–15%. Mounjaro usually costs more per month. Side-effect profiles are similar, and both are prescription-only weekly injections.

A clinician's side-by-side comparison of tirzepatide and semaglutide — efficacy, side effects, cost in India and who each suits — from Dr. Sanjay Sachdev (MBBS, MNAMS), consulting physician in South Delhi.

Trial figures are averages, not promises; individual response varies widely. Costs are indicative Indian pharmacy ranges for 2026.

How they differ mechanically

Semaglutide is a GLP-1 receptor agonist: it slows gastric emptying, blunts appetite and improves insulin response. Tirzepatide adds GIP receptor activity, which in head-to-head diabetes trials produced larger falls in both weight and HbA1c at comparable tolerability.

Choosing between them in real life

Four practical questions decide it: how much weight you need to lose, whether you have Type 2 diabetes, what you can sustain financially for 12 months, and what your pharmacy can actually supply without gaps. A drug you have to stop mid-titration because of supply or cost is the wrong drug.

Switching, stalling and plateaus

Plateaus are normal after six to nine months. Before switching molecules we review protein intake, resistance training, sleep, alcohol, thyroid function and other medicines — a plateau caused by muscle loss and under-eating protein does not improve by changing the injection.

Safety screen that applies to both

We exclude personal or family history of medullary thyroid cancer or MEN2, active pancreatitis, severe gastroparesis and pregnancy, and review gallstones, retinopathy and kidney function. Diabetic patients on insulin or sulfonylureas need those doses reduced to avoid hypoglycaemia.

Frequently asked

8 answers
Which is better, Mounjaro or Ozempic?
For weight loss, tirzepatide (Mounjaro) has stronger trial data — about 15–22% body-weight loss versus 10–15% for semaglutide. For diabetes control both work well. 'Better' in practice means whichever you tolerate, can afford monthly, and can buy reliably.
What is the actual difference between the two drugs?
Semaglutide acts on the GLP-1 receptor. Tirzepatide acts on both GIP and GLP-1 receptors, which appears to give greater appetite suppression and metabolic effect at equivalent tolerability.
Is Mounjaro worth the extra cost over Ozempic?
If your target is 15% or more of body weight, or semaglutide has plateaued for you, usually yes. If you need 5–10% loss or mainly better HbA1c, semaglutide — or oral Rybelsus — often achieves it for less.
Can I switch from Ozempic to Mounjaro?
Yes, and it is a common step when weight loss stalls or nausea is unmanageable. Switching is not dose-for-dose: we restart tirzepatide at 2.5 mg and re-titrate, with a short overlap plan so appetite control does not disappear.
Which one has fewer side effects?
Broadly comparable — nausea, reflux, constipation or loose stools, early satiety, all worst in the first days after each dose step. Rate of titration, hydration, fibre and meal size matter more than which molecule you use.
Do both need a prescription in India?
Yes. Both are prescription-only injectables, and both need supervision for dose titration, contraindication screening and monitoring of kidney function, gallbladder and thyroid symptoms.
Which is better if I have Type 2 diabetes?
Both are licensed for Type 2 diabetes. Tirzepatide generally gives a larger HbA1c fall. Choice also depends on kidney function, cardiovascular history, current insulin or sulfonylurea use, and cost sustainability.
Who decides which one I start on?
Dr. Sanjay Sachdev (MBBS, MNAMS) reviews your BMI, HbA1c, comorbidities, medicines, budget and local supply, then sets a titration plan. Consultations at Friends Colony East and Maharani Bagh, New Delhi 110065 — +91 98110 78388.
