Mounjaro Side Effects — What to Expect and Manage

Most Mounjaro (tirzepatide) side effects are digestive: nausea, early fullness, constipation or diarrhoea, reflux and burping, worst in the days after each dose increase and usually settling within one to three weeks. Serious but uncommon risks include pancreatitis, gallbladder disease and dehydration.

A practical guide to tirzepatide side effects — what is expected, how each is managed, and the red flags needing same-day review — from Dr. Sanjay Sachdev (MBBS, MNAMS), consulting physician in South Delhi.

The titration weeks

Mounjaro starts at 2.5 mg for four weeks, then 5 mg. Symptoms peak in the two to four days after each step. We only escalate when you are comfortable at the current dose, which is why many patients settle at 5–10 mg rather than 15 mg.

Managing symptoms without quitting

Smaller and slower meals, less fat and fried food, no lying down after eating, adequate water and salt, fibre or an osmotic laxative for constipation, and a short antacid course for reflux. If symptoms persist we hold or step back a dose rather than push on.

Red flags — stop and call the same day

Severe constant upper abdominal pain radiating to the back; right-upper abdominal pain with fever or jaundice; vomiting that prevents drinking; fainting or repeated low sugars; a new neck lump, hoarseness or trouble swallowing.

Monitoring while you are on it

Baseline and periodic HbA1c, lipids, liver and kidney function, thyroid, vitamin D and B12, plus weight, waist and blood pressure. Diabetic patients on insulin or sulfonylureas have those doses reduced up front to avoid hypoglycaemia.

Frequently asked

8 answers
What are the common Mounjaro side effects?
Nausea, reduced appetite, early fullness, constipation or diarrhoea, reflux, burping, and mild fatigue. They cluster in the days after each dose increase and typically settle within one to three weeks at a stable dose.
Is tirzepatide harder to tolerate than semaglutide?
In practice, no — trial rates of nausea are broadly similar. How fast the dose is escalated matters far more than which molecule you take. Most intolerance we see comes from jumping strengths every four weeks regardless of symptoms.
What are the serious risks with Mounjaro?
Pancreatitis (severe constant upper abdominal pain going through to the back), gallbladder disease (right-upper pain, fever, jaundice), dehydration from persistent vomiting, and hypoglycaemia when combined with insulin or sulfonylureas. Rare thyroid C-cell signals mean we avoid it with a family history of medullary thyroid cancer or MEN2.
How do I stop the nausea?
Halve portion sizes, eat slowly, cut fried and high-fat meals, avoid lying down after eating, keep fluids and fibre up, and hold the current dose an extra two to four weeks. A short antacid course helps reflux. Escalating through symptoms rarely ends well.
Will I lose muscle on Mounjaro?
You can, if weight falls fast without adequate protein and resistance training. We set a protein target by body weight, add strength work twice a week, and slow the rate of loss if strength or lean mass is dropping.
Can I drink alcohol on tirzepatide?
Small amounts are usually tolerated, but alcohol worsens nausea and reflux, adds empty calories, and raises pancreatitis and hypoglycaemia risk. Most patients find their tolerance drops sharply anyway.
What happens if I miss a weekly dose?
Take it as soon as you remember if it is within four days; otherwise skip it and resume your usual day. Never double up. Tell us if you miss more than one dose — appetite control returns unevenly and nausea can restart.
Who should not take Mounjaro?
Anyone with a personal or family history of medullary thyroid cancer or MEN2, previous pancreatitis, severe gastroparesis, or who is pregnant, planning pregnancy or breastfeeding. We screen for all of these before prescribing.
