Medical fitness certificates issued after a proper examination by Dr. Sanjay Sachdev (MBBS, MNAMS) — for employment and joining, school and college admission, sports and gym, visa and immigration, driving licence, and fitness-to-resume-work after illness or surgery. Friends Colony East and Maharani Bagh, Monday to Saturday.
Employment and pre-joining fitness, government and PSU appointment formats, school, college and university admission, sports and gym enrolment, trekking and adventure travel clearance, driving licence fitness, fitness-to-fly, and fitness-to-resume-duty after illness, surgery or hospitalisation.
What the examination covers
History and medication review, height, weight and BMI, pulse and blood pressure, vision check, and systemic examination of the chest, cardiovascular system, abdomen and nervous system. Findings are documented — this is a genuine assessment, not a rubber stamp.
Tests added when the format requires them
Complete blood count, blood sugar or HbA1c, urine routine, ECG, chest X-ray, vision and colour vision testing, and hepatitis or immunity status where the requesting body specifies. Home sample collection is available across South Delhi if you would rather not travel twice.
Bring your own form
Employer, university, embassy, sports federation and gym formats are completed, signed and stamped with the doctor's registration number. WhatsApp the form to +91 98110 78388 in advance and we will confirm which tests to arrange before you come in.
Visa and immigration medicals
General fitness certification for visa and immigration applications, including declarations of current health status, chronic conditions and medication. Note that some countries mandate a panel-appointed physician — check your application before booking, and we will advise if you need a designated panel clinic instead.
Eligibility — who can be certified
Adults are examined directly; a parent or guardian must accompany anyone under 18. Fitness is determined by the examination, not by age or history alone — well-controlled diabetes, hypertension, thyroid disease, asthma or a healed fracture are recorded with current status and medication rather than treated as automatic disqualifiers. Where a finding genuinely affects the role, restrictions are stated clearly instead of the certificate being withheld.
Documents to bring
Government photo ID, two passport-size photographs, the requesting body's original printed form, the offer, joining or admission letter, and your previous prescriptions, reports or discharge summary if you have a chronic condition or recent hospitalisation. Visa applicants should also carry the passport, embassy form and vaccination record; students the institution proforma and immunisation card; sportspersons the federation form plus any earlier ECG, echo or injury records.
Same-day appointments
Both clinics run Monday to Saturday, 10:00 AM – 7:00 PM by appointment. Fitness examinations are usually accommodated the same or next day — useful when a joining date or admission deadline is close.
Frequently asked
13 answers
Who can issue a medical fitness certificate?+
A registered medical practitioner (MBBS or above) after an actual examination. Dr. Sanjay Sachdev (MBBS, MNAMS) issues fitness certificates at Friends Colony East (C-58) and Maharani Bagh (E-64) following a documented consultation — history, vitals, BP, systemic examination and any tests the requesting body specifies.
What is a medical fitness certificate used for?+
Employment and joining formalities, government and PSU appointments, school, college and university admission, sports participation, gym enrolment, visa and immigration requirements, driving licence, adventure travel and trekking permits, and fitness-to-fly or fitness-to-resume-work after illness or surgery.
What does the fitness examination involve?+
Medical history and medication review, height, weight, BMI, pulse, blood pressure, vision check, and examination of the chest, heart, abdomen and nervous system. Depending on the requirement, blood counts, blood sugar, chest X-ray, ECG, urine routine or vision testing may be added.
How long does it take to get the certificate?+
The examination takes 20–30 minutes and the certificate is issued the same visit when no additional tests are required. If blood work, ECG or an X-ray is specified by the employer or authority, the certificate follows once those reports are reviewed — usually the same or next day.
Do you issue fitness-to-work certificates after illness or surgery?+
Yes. Fitness-to-resume-duty after illness, surgery, fracture, dengue or a hospital admission is assessed on recovery status, current medication, and the physical demands of the role, with restrictions or a phased return specified where appropriate.
Who is eligible for a medical fitness certificate?+
Anyone aged 18 and above can be assessed directly; minors need a parent or guardian present for consent. Eligibility is decided by the examination itself — you are certified fit if vitals, systemic examination and any specified tests are within the range the requesting body accepts for the role. Controlled diabetes, hypertension, thyroid disease or asthma do not automatically disqualify you, but they are recorded on the certificate with current medication and control status.
What documents do I need to bring?+
A government photo ID (Aadhaar, passport, PAN or driving licence), two passport-size photographs, the employer, university, embassy, sports body or gym form in its original printed format, any offer or joining letter that states the medical requirement, and your previous prescriptions or discharge summary if you have a chronic condition or recent hospitalisation.
What extra documents are needed for a visa or immigration medical?+
Passport with the visa application or reference number, the exact medical form issued by the embassy or consulate, your vaccination record (COVID-19, hepatitis B, MMR, tetanus as applicable), and earlier chest X-ray or blood reports if you have a history of TB or chronic illness. Confirm whether your destination country mandates a panel-appointed physician before booking.
What do students and sportspersons need to bring?+
For school, college or university admission: the institution's medical proforma, admission or allotment letter, ID proof, immunisation record and blood group report where asked. For sports or gym clearance: the federation, academy or gym form, details of any past injury, fracture, surgery, fainting episode or cardiac symptom, and prior ECG or echo reports if you have been advised one before.
What is required for a fitness-to-resume-work certificate?+
Your discharge summary or treating specialist's notes, current prescription list, recent investigation reports, and a note from your employer describing the physical demands of the role — desk work, shift duty, driving, heavy lifting or travel. This lets us certify fitness accurately or specify restrictions and a phased return where recovery is still in progress.
Can someone else collect the certificate on my behalf?+
The examination must be done in person — no certificate is issued without an actual assessment. Once issued, a family member can collect the signed and stamped copy with your written authorisation and their own ID proof, or we can send a scanned copy on WhatsApp to +91 98110 78388 and keep the original at the clinic for pickup.
How much does a medical fitness certificate cost in Delhi?+
The charge is a standard consultation fee plus any tests the requesting organisation specifies. Current consultation and test charges are listed on the pricing page. WhatsApp +91 98110 78388 with the exact form or format your employer or authority requires.
Can you fill an employer's or embassy's own form?+
Yes — bring the printed form. Employer, university, embassy, sports body and gym formats are all completed and signed with the doctor's registration number and clinic stamp. Send the form on WhatsApp beforehand so we can tell you which tests to arrange first.
