Dr. Sanjay Sachdev (MBBS, MNAMS) manages thyroid disorders as part of his internal medicine practice — hypothyroidism, hyperthyroidism, Hashimoto's thyroiditis, subclinical thyroid disease, pregnancy thyroid and thyroid nodule follow-up. Two South Delhi clinics: Friends Colony East and Maharani Bagh.
For 90% of thyroid patients, an internal medicine physician is the right specialist — TSH-based dosing, symptom review, T3/T4 balance, antibody testing, and interpretation in the context of other conditions (diabetes, cholesterol, weight, mood). Endocrinology referral is reserved for complex cases: pituitary disease, adrenal involvement, or difficult nodules.
Thyroid rarely travels alone — most patients also have weight, cholesterol, BP or mood symptoms. A senior physician treats the whole picture, avoids over-medication (a common problem in Delhi practice), and follows the same patient over years.
Not every fluctuating TSH needs a prescription. This is the practical filter used in the practice.
Usually retest in 6–8 weeks with antibodies before starting treatment. Many patients don't need medication.
Book within a week. Start supervised replacement and re-check in 6 weeks.
Book within a week. Needs exam + ultrasound; most nodules are benign but every new one is checked.
Don't stop with thyroid. Screen iron, B12, vitamin D, sugar and sleep — often the real culprits.

For the vast majority of thyroid patients, a senior internal medicine physician is the right doctor. Endocrinology is reserved for complex cases.
Once stable on medication, every 6–12 months is sufficient. On a dose change, retest in 6–8 weeks. In pregnancy, every trimester.
Yes, at the correct dose. Most 'thyroid symptoms' on treatment are actually over- or under-replacement — worth reviewing the dose rather than switching brands.
TSH, free T4 and free T3 as a baseline. Anti-TPO antibodies if not tested before. If you have a nodule, an ultrasound report on hand is helpful.
It can contribute, but rarely alone. Iron, vitamin D, B12, sleep and sugar all matter — one visit typically covers all of them together.
