Vaccines for adults with chronic disease
Diabetes: influenza annually, pneumococcal from diagnosis, hepatitis B, shingles from 50.
Heart failure or coronary disease: influenza annually — it measurably reduces cardiac events — plus pneumococcal.
COPD or asthma: influenza and pneumococcal.
Chronic kidney disease or on dialysis: hepatitis B (double dose schedule with titre check), influenza, pneumococcal.
Chronic liver disease: hepatitis A and B, influenza, pneumococcal.
Splenectomy or asplenia: pneumococcal, meningococcal, Hib, influenza — this group needs the fullest cover.
Travel and occupational vaccines
Planned travel changes the list: hepatitis A and typhoid for most of South and South-East Asia and Africa; yellow fever (certificate required) for parts of Africa and South America; meningococcal ACWY for Hajj and Umrah, mandatory; rabies pre-exposure for veterinarians, animal handlers and long-stay rural travel; Japanese encephalitis for rural endemic areas. Plan 4–6 weeks before departure so multi-dose courses can be completed.
Catch-up when records are lost
Missing childhood records are the norm rather than the exception in adults. We either give a full catch-up schedule assuming no immunity, or test titres where that is cheaper and clinically meaningful — hepatitis B surface antibody, measles IgG, varicella IgG. You leave with a written personal immunisation card listing what was given, the batch numbers and the date of the next dose.
Safety, side effects and what to expect
Expect a sore arm for a day or two, and occasionally mild fever or fatigue for 24–48 hours — most pronounced after the shingles and Tdap vaccines, and a sign the immune system is responding. Serious reactions are rare; you are observed for 15 minutes after each injection. Tell us about egg allergy (relevant for some influenza vaccines), immunosuppressive treatment, recent immunoglobulin, pregnancy or planned pregnancy.

Do adults with diabetes need extra vaccines?+
Yes. Diabetes raises the risk of severe pneumonia, influenza complications and shingles. The priority set is annual flu, pneumococcal (at any age from diagnosis, not waiting until 65), hepatitis B (adults with diabetes have a higher risk of acquisition), shingles from 50, and Td/Tdap booster. This is one of the highest-value, lowest-cost interventions in chronic disease care.

Can I take more than one vaccine on the same day?+
Yes — inactivated vaccines can be given on the same visit at different injection sites, which is how most catch-up schedules are completed. Live vaccines (MMR, varicella) are either given together or separated by four weeks. Dr. Sachdev builds a schedule so you make the fewest possible visits.
Which vaccines are important before pregnancy or in pregnancy?+
Before conception: MMR and varicella if non-immune (both need a one-month gap before pregnancy), plus hepatitis B and HPV catch-up. During pregnancy: influenza in any trimester and Tdap between 27 and 36 weeks to protect the newborn against whooping cough. Live vaccines are avoided in pregnancy.
Do I need a shingles vaccine if I already had chickenpox?+
Yes — that is exactly who it is for. Shingles is the reactivation of the chickenpox virus that has stayed dormant since childhood, so anyone who has had chickenpox carries the risk. The recombinant two-dose vaccine is recommended from age 50 and is over 90% effective at preventing shingles and post-herpetic neuralgia.
What if I have lost my childhood vaccination record?+
This is common and not a problem. Where records are missing, we either assume no protection and give a catch-up schedule, or check antibody titres where that is cheaper and clinically useful (hepatitis B surface antibody, measles IgG, varicella IgG).
