What the HbA1c number means
Below 5.7% normal. 5.7–6.4% pre-diabetes — the stage where lifestyle change and, in selected patients, metformin or a GLP-1 prevents progression. 6.5% and above confirms diabetes on two readings. Each 1% reduction in HbA1c meaningfully lowers the risk of kidney, nerve and eye complications, which is why the target is individualised rather than universal.
How often to test — and where money gets wasted
Every 3 months after a medication change or when above target; every 6 months when stable. Red cells survive around 90 days, so an HbA1c repeated at 4 weeks tells you almost nothing new. Most of the over-spending we see in diabetes care is monthly HbA1c testing paired with no review of the medication list.
When HbA1c is not the right test
Anaemia, iron and B12 deficiency, chronic kidney disease, recent transfusion, pregnancy and haemoglobin variants (including thalassaemia trait, which is common in North India) all shift HbA1c. In those cases fructosamine, a 7-point glucose profile or CGM is the accurate option. A CBC alongside the HbA1c is what catches this.

What is the HbA1c test price in Delhi?+
An HbA1c test costs ₹500–₹800 at NABL-accredited labs in South Delhi. Bundled into a diabetes review panel with fasting sugar, lipid profile, kidney function with eGFR and urine microalbumin, the whole set costs ₹2,800–₹3,800 including home collection and a physician review.
How often should HbA1c be tested and what does that cost per year?+
Well-controlled type 2 diabetes: every 6 months — roughly ₹1,000–₹1,600 a year. Recently changed medication or HbA1c above 7.5%: every 3 months — ₹2,000–₹3,200 a year. Pre-diabetes: once a year. Testing HbA1c monthly is a waste of money because red cells live about 3 months, so the number cannot meaningfully move faster than that.
Do I need to fast for an HbA1c test?+
No. HbA1c reflects the last 8–12 weeks of average glucose, so you can give the sample at any time of day, after food. If a fasting glucose or lipid profile is being done at the same time, then fast 10–12 hours for those.
What HbA1c number is normal, pre-diabetic and diabetic?+
Below 5.7% is normal, 5.7–6.4% is pre-diabetes, and 6.5% or above on two occasions confirms diabetes. For most adults on treatment the target is below 7%, relaxed to 7.5–8% in the elderly or where hypoglycaemia is risky, and tightened toward 6.5% in young patients with short-duration diabetes.
Can HbA1c be falsely high or low?+
Yes — anaemia, iron or B12 deficiency, recent blood transfusion, kidney failure, pregnancy and haemoglobin variants all distort HbA1c. In those situations a fructosamine test or continuous glucose monitoring gives a truer picture. This is exactly why the number needs to be read alongside a CBC and clinical context, not in isolation.
Is home collection available for HbA1c?+
Yes, at no extra charge, across Friends Colony, New Friends Colony, Maharani Bagh, Defence Colony, Greater Kailash, Lajpat Nagar, Kalkaji, Saket, Vasant Kunj and Noida. Because HbA1c needs no fasting, collection can be booked at a time that suits you.
What does a diabetes review with Dr. Sachdev include?+
HbA1c interpreted against your home glucose readings, medication review (including whether a GLP-1 such as Ozempic or Mounjaro is appropriate), blood pressure and lipid targets, kidney and eye screening intervals, foot examination and a written plan with the date of the next test.
How much does diabetes management cost overall?+
Testing is the small part — roughly ₹4,000–₹6,000 a year for HbA1c, lipids, kidney function and urine microalbumin. Medication cost varies widely; a structured review often reduces it by removing duplicated drugs and switching to equally effective, cheaper alternatives.
